Apply for a Dog

The process to get a Guide or Assistance Dog from us has a number of steps. We  will want to know about you, your living arrangements and family or other supports. We also want to meet with you in your home so we know what tasks you require in a Guide or Assistance Dog. We also want a report completed by your medical professional detailing your need. However, first you will have to complete the following form and submit it to us. Make sure all of the areas are completed. Next, we’ll follow up with you personally to detail what will happen next.
